  • The focus of the 10 bed unit is to help patients regain their independence after the acute phase of an illness with the expectation that they will be able to return home.
  • For suitable candidates, the program serves as an alternative option to applying for placement in a Long Term Care facility.
  • Patients care for themselves (dressing, bathing, taking medication) with assistance if required, and participate in daily activities designed to regain strength and independence.

  • The support team includes an Occupational Therapist, Physiotherapist, Nurses, Personal Support Workers (PSW) and an Occupational Therapy/Physiotherapy Assistant.

Visiting Hours

2:00-4:00 PM, 7:00-8:00 PM

How do I access this service?

Patients from all of the hospital sites in Grey and Bruce Counties are screened by Community Care Access Centre (CCAC) Case Managers for program eligibility.
