Appointments are now required for Community Lab Collections. Please arrive no earlier than 5 minutes before your appointment. 
To book an appointment, call Patient Registration between the hours of 7:30 AM-3:30 PM at the site you wish to come to. Please have your health card and lab requisition ready when calling.

How do I access this service?
A physician referral is required.

*Note: standing orders are not kept on file.  Please bring a copy of your requisition and your health card.

Where do I register for my laboratory work?
If you have a Ministry of Health requisition (shown below), you do not need to register to have your samples collected.  Please proceed directly to the Community Lab Collections room.

If you have a South Bruce Grey Health Centre requisition (shown below), please visit Patient Registration to register when you arrive at the hospital.

Test Preparation
Please follow any preparations as advised by your physician (i.e. fasting). Test preparations are also indicated on the hospital laboratory request form. If you have any questions, please contact the laboratory.
