  • Be treated with dignity and respect and free from any form of abuse.
  • Have my privacy and confidentiality respected.
  • Have open and ongoing communication with my care team.
  • Receive timely information about my diagnosis and treatment.
  • Have support person(s) involved in my care, if desired.
  • Have access to my personal health information.
  • Receive timely information about managing my care after discharge.
  • Have opportunities to incorporate cultural practices and spiritual beliefs in my care.

My responsibilities as a patient are to:

  • Communicate clearly and openly about my care and my needs.
  • Actively participate in decision-making about my care.
  • Provide the care team with accurate information about my health.
  • Understand that verbal or physical abuse will not be tolerated.
  • Understand that, at times, I may need to wait while staff attend to other patients with more urgent care needs.
  • Report safety risks to the care team.
  • Abide by hospital policies and code of conduct.
  • Respect the privacy and confidentiality of other patients, including no photography or videotaping without consent.
