- Have support person(s) involved in my care, if desired.
- Have access to my personal health information.
- Receive timely information about managing my care after discharge.
- Have opportunities to incorporate cultural practices and spiritual beliefs in my care.
My responsibilities as a patient are to:
- Communicate clearly and openly about my care and my needs.
- Actively participate in decision-making about my care.
- Provide the care team with accurate information about my health.
- Understand that verbal or physical abuse will not be tolerated.
- Understand that, at times, I may need to wait while staff attend to other patients with more urgent care needs.
- Report safety risks to the care team.
- Abide by hospital policies and code of conduct.
- Respect the privacy and confidentiality of other patients, including no photography or videotaping without consent.
